About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role includes reviewing inpatient admission records to support appropriate utilization of hospital resources and determine whether admissions meet the appropriate level of care. The Case Manager assesses discharge needs, coordinates discharge plans, and works collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The position also involves maintaining and enhancing payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.

Nice To Haves

State licensure where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
