About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role reviews inpatient admission records to ensure appropriate utilization of hospital resources and determine whether admissions are at the appropriate level of care. The Case Manager assesses discharge needs and coordinates appropriate discharge plans, collaborating with the interdisciplinary team to support desired financial and quality outcomes. The role also maintains and enhances payor relationships.

Requirements

Current BLS certification.
Three to five years of inpatient experience.

Nice To Haves

State licensure where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
