About The Position

The ACT Rehab Case Manager provides variable levels of support to consumers with mental health issues as part of an integrated Psychosocial Rehab treatment team. The role includes helping consumers access medical, social, educational, and other appropriate support services; connecting them to more intensive services when needed; and conducting frequent monitoring, needs assessments, service planning, and coordination. The position also administers TIMA scales and performs other TIMA medication-management functions. The position provides Psychosocial Rehabilitative services within Level of Care 4 of the Texas Resiliency and Recovery model. It ensures that Recovery Plans are developed based on ANSA calculations and the Authorized Level of Care. The work involves frequent in-person, long-term contact with individuals, families, and service providers, as well as direct assessment of consumers’ living environments and spontaneous assistance as needs arise. Rehabilitative services address daily and independent living skills, including problem solving, accessing services and community resources, obtaining and maintaining safe, affordable housing, effective communication, appropriate socialization, and developing natural support systems. The role requires identifying needs, working professionally under pressure, completing thorough assessments and documentation, locating and using community resources, advocating and negotiating with outside service entities, and developing positive relationships with consumers. Duties follow established policies, procedures, and ACT best-practice guidelines, with some latitude for personal judgment. The case manager serves on the 24-hour ACT On-Call Crisis Team, providing crisis resolution and intervention throughout the Center’s catchment area to people on the ACT Team caseload, and must consistently meet monthly performance targets. The schedule is Monday through Friday, 8 a.m. to 5 p.m. Duties are performed under the supervision of the ACT Team Mental Health Program Administrator.
