About The Position

The ACT CADC Case Manager is responsible for providing comprehensive, community-based services to adults living with serious mental illness, utilizing the Assertive Community Treatment (ACT) model. This position focuses on providing therapeutic support, assessment, and treatment planning for clients struggling with substance use disorders, recovery-oriented care, coordination of services, and ongoing support to help individuals achieve stability and independence within the community.

Requirements

  • Bachelor's Degree in Social Work, Psychology, Counseling, or related human services field (Master’s preferred).
  • Two (2) years’ experience working in a community mental health or behavioral health setting preferred.
  • Valid driver’s license, reliable vehicle and ability to travel throughout the service area is required.
  • Valid and current CADC certification for the State of Illinois is required.
  • Working knowledge of DSM-5 diagnostic criteria and ASAM levels of care.
  • Familiarity with community-based mental health services, including Medicaid-funded programs.
  • Strong organizational, written, and verbal communication skills.
  • Computer proficiency, including electronic health records and Microsoft Office applications.
  • Ability to work effectively in a fast-paced, team-based environment.
  • Cultural competency and sensitivity to diverse populations.

Responsibilities

  • Meet a total of 6 to 7 billable encounters/engagement with clients per day.
  • Participate in a rotating on-call schedule to provide 24/7 crisis intervention and support.
  • Collaborate with State Mental Health Facilities to coordinate discharge planning for high-utilization individuals.
  • Engage and maintain close coordination with the ACT team and community support services.
  • Develop and maintain professional working relationships with internal and external referral sources.
  • Conduct comprehensive biopsychosocial assessments to determine service needs and level of care.
  • Develop, implement, and regularly update IM+CANS and additional supporting clinical documentation with client and/or family input.
  • Link clients to necessary services, including medical, psychiatric, housing, vocational, financial, and recreational resources.
  • Provide housing navigation, placement assistance, and tenancy support as needed.
  • Deliver strengths-based, person-centered case management, supportive counseling, and problem-solving interventions.
  • Engage in proactive outreach and engagement to maintain client contact and prevent hospitalizations.
  • Participate in interdisciplinary team meetings to ensure coordinated care and individualized treatment planning.
  • Ensure timely documentation of all client interactions in accordance with Medicaid, agency, and regulatory requirements.
  • Conduct DIS (Direct Individual Service) billing for all services provided in a timely and accurate manner.
  • Provide public education and community outreach on mental health needs and services as assigned.
  • Maintain all required credentials and engage in continuing education/professional development.
  • Perform other duties as assigned by the Program Supervisor or Department Leadership.
