About The Position

This is an entry-level professional position in community-based psychosocial rehabilitation services for individuals with serious and persistent mental illness. The Case Manager plans, coordinates, provides, and monitors therapeutic, rehabilitative, support, and community integration services for adults with severe psychiatric disability. These services are designed to assist individuals in integrating as independently and fully into the life of the community as possible. The Case Manager works independently and must demonstrate good clinical judgment, decision-making, time management, and appropriate treatment for consumers in the community.

Requirements

• Bachelor’s Degree in psychology, social work, psychosocial rehabilitation, or related human services field, OR Associate’s degree in a related field and two years’ experience working with individuals with mental illness.
• Understanding of mental illness and a desire to reduce stigma and effectively support this population.
• Valid driver’s license.

Responsibilities

• Transports clients as needed.
