About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role reviews inpatient admission records to assess appropriate use of hospital resources and determine whether patients are admitted at the appropriate level of care. The Case Manager assesses discharge needs and coordinates appropriate discharge plans, working with the interdisciplinary team to support desired financial and quality outcomes. The role also maintains and enhances relationships with payors.

Requirements

Current BLS certification.
Three to five years of inpatient experience.

Nice To Haves

State licensure in the state where the hospital resides is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
