About The Position

The Case Manager coordinates care across the interdisciplinary team for a designated patient population. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether admissions meet the appropriate level-of-care criteria. The Case Manager assesses discharge needs, coordinates suitable discharge plans, and works collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The position also maintains and enhances relationships with payors.

Requirements

• Current BLS certification.
• Three to five years of inpatient experience.

Nice To Haves

• State licensure where the hospital resides is preferred.
• Experience in an acute, IRF, or LTACH setting is preferred.
• Professional discipline may include occupational therapy, physical therapy, registered nursing, respiratory therapy, speech therapy, or licensed social work.
