About The Position

The Case Manager coordinates care across disciplines for a designated patient population. The role reviews inpatient admission records to assess utilization of hospital resources and whether admission is appropriate for the patient’s level of care. The Case Manager identifies discharge needs, coordinates discharge plans, and works with the interdisciplinary team to achieve desired financial and quality outcomes. The position also maintains and enhances relationships with payors.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

Licensure in the state where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.

Responsibilities

Coordinate interdisciplinary care for a designated patient population.
Review inpatient admission records to assess utilization of hospital resources and determine whether admission is appropriate for the level of care.
Assess and identify discharge needs and coordinate an appropriate discharge plan.
Collaborate with the interdisciplinary team to facilitate desired financial and quality outcomes.
Maintain and enhance payor relationships.
