Under the supervision of the Community Services for Families Program Supervisor, the Case Manager will work with a caseload of culturally diverse families who are engaged in improving their parenting skills. The role provides prevention and intervention support services through in-home trauma-informed, family-centered, evidence-based, and culturally competent services, prioritizing referrals received from Child and Family Well-Being (CFWB). The Case Manager delivers in-person parenting education to families identified as "at risk" for child abuse and neglect and referred by Child and Family Well-Being (CFWB), formerly CWS, and community partners. The position assists families in developing and achieving parenting goals as identified on their CFWB case plan, by utilizing a strength-based approach and trauma-informed practice. The role delivers one of two evidence-based curriculums for parenting: STEP and/or SafeCare as directed for each family. The Case Manager performs intakes, assessments, and case management of clients as required by contract objectives. They assess families to determine their basic needs such as medical care/insurance, food stamps, and information on nutrition and childhood obesity, and provides referrals as needed. The position participates in Child Family Team Meetings to discuss client progress in services and support them in achieving their case plan goals. The Case Manager participates in the quarterly CSF peer review process to discuss service delivery and improve best practices, and maintains ongoing communication with the CSF team to ensure quality of service delivery. They maintain detailed case files and documentation using county-approved forms and formats and produces monthly reports of activities, submitting them to the CSF supervisor. The role participates in required staff meetings, collaborative meetings, supervisions, trainings, county meetings, QA process, and other meetings as required by the program and contract objectives. The Case Manager participates in CFWB work groups and committees as requested by the CSF Manager. The position provides program services in both English and Spanish, and interprets documents as needed to fit the needs of the clients. The Case Manager effectively completes assigned collateral responsibilities as designated in work agreements and other duties as assigned.
