About The Position

The Case Coordinator works in a highly technical, fast-paced, and challenging environment, collaborating with multidisciplinary team members to provide excellent patient care. In conjunction with and in support of the physician-led multidisciplinary care team, the Case Coordinator assures appropriate use of clinical resources to achieve optimal clinical outcomes.

Requirements

• Associate degree in nursing; applicants with insurance or hospital case management experience may be considered.
• Current license to practice professional nursing in Texas.
• Non-nurse professionals must hold a license from the respective agency.
• At least 2 years of health care experience.
• At least 1 year of acute hospital experience.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Certified Case Manager (CCM) certification in case management preferred.
• Acute hospital case management experience preferred.
