Care Manager (CNA/CHHA) Part Time

About The Position
The Care Manager is a part-time, non-exempt, hourly position reporting to the Health & Wellness Director. The role assists residents with aspects of daily life according to their care plans, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The position has all shifts available and pays $21.00–$23.00 per hour.
Requirements
- Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) certification.
- Ability to perform the physical demands of the position, including lifting or carrying up to 50 pounds and intermittent lifting and supporting of residents.
- Use proper body mechanics.

Responsibilities
- Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in each resident’s care plan.
- Maintain residents’ safety, comfort, dignity, and confidentiality.
- Escort residents to meals and activities.
- Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
- Document all services provided to assigned residents during the shift.
- Immediately report any change in a resident’s condition to a supervisor.
