About The Position

Care Coordinators help children with chronic medical or behavioral health conditions connect with the medical, social, educational, and community services they need to stay healthy and avoid unnecessary emergency room visits. They learn about each child and family’s needs, work with members and other care providers to develop person-centered care plans, and help members take steps to access and use services, keep appointments, and adhere to recommended treatments. The role works in a team setting and primarily in the field. Care Coordinators assess members’ risks and needs, coordinate referrals and appointments, provide education, monitor service access and care transitions, and collaborate with families, caregivers, providers, schools, and community organizations.
