About The Position

New Horizons In-Home Care has provided care to the Oregon community for over 30 years, serving older adults, seniors, individuals with disabilities, and medically fragile children. The Care Coordinator is responsible for timely scheduling, coordinating, and recording new client assessments; developing client care plans; reporting; and providing ongoing assessments for existing clients. The role calls for demonstrating the agency’s culture of compassion, accountability, and results through integrity, follow-through, consistency, customer service, and commitment to client satisfaction. The position also participates in an on-call rotation and provides phone support and caregiving assignments.

Requirements

• At least one year of customer service experience in a healthcare-related field.
• Be a self-starter who can identify opportunities and recommend solutions.
• Creative problem-solving skills.
• Strong written, verbal, and presentation skills with diverse populations.
• Ability to multitask, remain calm, and work as a team player in a fast-paced environment while maintaining attention to detail.
• Strong computer and typing skills.

Responsibilities

• Perform initial assessments for clients with external Case Managers, including Medicaid, VA, OPI, and ODDS.
• Perform quarterly assessments for all non-nursing clients.
• Complete new client care plans and task lists, coordinating with the New Horizons RN for clients receiving nursing services.
• Evaluate and report the quality of services provided at client sites.
• Identify client physical, nutritional, and hygiene support needs, including transfer and mobility needs.
• Develop home safety plans.
• Participate in the on-call rotation, provide phone support, and make caregiving assignments.
• Maintain the confidentiality of client, employee, and agency operations.
