The Care Coordinator- Outreach is responsible for developing and providing high quality therapeutic interventions and services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. This position is field based traveling throughout the Alameda county. This is a community-based position providing direct, face-to-face services to individuals who are chronically homeless and travel throughout Alameda County is required. Full time positions | Mond-Friday (8:00 - 4:30 pm) Locations: Oakland, Berkeley, Fremont (1) Oakland HRC outreach position: required to provide HRC/Coordinated Entry services in the community while holding small caseload in designated areas (1) Mobile Access Point outreach position East county: holds small caseload of individuals through outreach efforts. Light case management. Preferably someone with knowledge of that area. (1) North County HRC outreach position Berkeley, Albany, Emeryville: same as Oakland HRC responsibilities (2) Fremont Navigation Center Outreach staff: Traditional outreach with main focus being on making direct referrals to the Fremont Navigation center
