About The Position
Puzzle Healthcare is a mission-driven, national post-acute care organization focused on protecting patients after hospital stays. The Nurse Practitioner or Physician Assistant will provide focused care in nursing facilities to help reduce readmissions and improve recovery. This onsite role supports one skilled nursing facility in Austin, Texas, with a flexible, part-time, RVU-based schedule of one to two days per week. The schedule is limited to weekday daytime hours, with no call or weekends. Documentation is completed using Puzzle’s EHR, often remotely from home.
Responsibilities
- Round in assigned facilities on a consistent weekday daytime schedule.
- Oversee daily patient interactions through timely micro visits, including focused assessments of recovery, pain management, wounds, and functional decline.
- Collaborate with physicians, therapists, and facility staff on individualized recovery plans.
- Use Puzzle’s EHR to complete documentation, often remotely from home.
- Monitor readmission risk and help close gaps in post-acute care.
- Maintain compliance with post-acute and PM&R-related regulations.
