About The Position

To support the Mission of Helio Health, Inc., the Adult Health Home Care Coordinator provides comprehensive care management and coordination of services to Health Home enrollees and eligible Medicaid or Medicaid Managed Care community members. The role is part of Helio Health’s In-Community program and is full-time. The position pays $21.50–$23.00 per hour and is eligible for a sign-on incentive of up to $1,500.

Requirements

• Associate's degree required, preferably in Human Service, Forensics, Psychology, Public Health, or a related concentration.
• Two years’ experience working with individuals with mental health, substance use, or co-occurring disorders.
• Valid driver's license and personal vehicle required for travel throughout the community.

• Bachelor's degree preferred.

Responsibilities

• Conduct outreach and assessments for Health Home services for individuals who are Medicaid eligible and/or have active Medicaid.
• Conduct eligibility screenings and Health Home enrollment.
• Collaboratively create a comprehensive, person-centered plan of care with enrollees to address their medical, behavioral health, and social service needs.
• Provide health promotion and education, and coordinate all aspects of an individual's care.
• Offer family support and provide referrals or linkages to community, social, and other support services as needed.
• Assist individuals in keeping appointments and monitor service delivery.
• Provide care management services at various locations, including community locations and enrollees’ homes.
