About The Position

The Admissions Liaison coordinates an efficient, compassionate admission process for inpatient and outpatient patients. The role handles referrals, verifies information, completes pre-certifications, prepares admissions consent packets, maintains an accurate patient listing, and facilitates scheduling and admission. The liaison also contributes to the referral process and enters statistical data into hospital systems. The position requires the candidate to be a RN, LPN, OT, PT, SLP, RT, or LSW.

Requirements

• Must be a RN, LPN, OT, PT, SLP, RT, or LSW.

Nice To Haves

• One or more years of experience in hospital admissions procedures.
• Licensure as a clinician.
• Current CPR certification.
• CRRN certification.
• A strong understanding of multidisciplinary care levels.
• At least one year of clinical experience.

Responsibilities

• Coordinate an efficient admissions process for all patients.
• Perform pre-certifications.
• Assemble admissions consent packets.
• Maintain an accurate listing of all patients in the hospital.
• Receive and verify referrals, gather essential information, and complete procedures needed for scheduling and admission.
• Contribute to the referral process.
