About The Position
This position will oversee both sites. Under the supervision of the Marketing Manager, obtains initial information relating to admissions, coordinates intake process, and provides information and referrals to community agencies as appropriate. Promotes the Senior CommUnity Care Program to potential participants and community agencies.

- Skilled nursing experience
Responsibilities
- Process and complete admissions
- Provide pre-admissions counseling, information and tours
- Review EPIC for proper documentation and orders
- Facilitate PAS Screening and Level II as needed
- Assist Social Services team with various tasks
- Participate in Vulnerable Adult investigations and complete necessary documents/reports
