About The Position

Requirements

• PGY1 plus 1–3 years of experience, or PGY2, or 3–5 years of experience in a clinical specialty area.
• Maintain certification and credentials in accordance with the Massachusetts Board of Registration in Pharmacy and comply with applicable laws, regulations, and institutional policies.
• Influenza vaccination is required as a condition of employment.

Responsibilities

• Collaborate with physicians, nurses, and other healthcare professionals as part of the multidisciplinary care team.
• Review and verify medication orders using patient-specific information, and clarify inappropriate, incomplete, or unclear orders with prescribers in a timely manner.
• Document and communicate pertinent information with healthcare team members and pharmacy staff to support continuity of care.
• Provide drug information to physicians, nurses, and other healthcare professionals.
• Check and verify sterile and non-sterile medications.
• Supervise students, technicians, and residents within the pharmacist’s area of practice.
• Promote patient safety by submitting medication-related events and near misses.
• Complete daily pharmacy surveillance monitoring, follow-up, documentation, patient-care initiatives, and relevant reports.
• Work collaboratively with pharmacy staff and uphold respect and teamwork across the Pharmacy Department.
• Precept pharmacy students and resident rotations, including resident-led quality improvement projects; may serve as Residency Program Coordinator or Residency Program Director.
• Participate in departmental initiatives, including formulary stewardship, Pharmacy & Therapeutics Committee presentations, policy and procedure development, workflow assessments, and development of departmental tools and resources.
• Support a core practice area through interdisciplinary education, ongoing specialty training, and service as a resource for Pharmacy staff.
• Update clinical guidelines and present hospital-wide educational initiatives.
• Serve on hospital interdisciplinary committees or workgroups to improve processes and quality outcomes.
• Support IRB-approved research activities, medication use evaluations, and cost-savings initiatives.
• Provide patient education and discharge counseling.
• Contribute to clinical policy, protocol, and treatment-guideline development and implementation.
• Support education, quality improvement initiatives, and research aligned with the Cancer Center’s mission.
• Perform other duties as assigned.
