About The Position

Requirements

• Graduate from an accredited school of nursing.
• Three years of experience in specialty-specific nursing in a hospital, clinic, or community setting.
• Demonstrated ability to manage a significant customer population on a weekly basis.
• Demonstrated broad clinical knowledge of specialty-specific nursing.
• Strong leadership, verbal, written, and interpersonal communication skills.
• Demonstrated proficiency in computer literacy or use of a variety of software programs, such as Excel and PowerPoint.
• Current licensure as an RN in the state of Minnesota.
• Current BLS certification.

Nice To Haves

• Bachelor’s degree in nursing.
• Experience in Nurse Navigation.

Responsibilities

• Provide individualized assistance to patients, families, and caregivers across the care continuum.
• Expedite and coordinate care and address health-system barriers.
• Coordinate patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes.
• Focus on high-risk patients and patients with complex care needs, including multidisciplinary care.
• Improve transitions in care and adherence to care.
• Act as a clinical advocate and educator for patients.
• Build and maintain community relationships to provide timely and reliable access to quality care.
• Identify and alleviate stressors during transitions and barriers to care.
