This position will support the Adult Care and Transition Team, providing psychosocial assessment, care coordination, resource navigation, and transition-to-adult-care support for adolescents and young adults with complex medical needs, chronic health conditions, and developmental disabilities. The social worker will partner with patients, families, healthcare providers, schools, and community agencies to address social determinants of health, facilitate access to services, and promote successful engagement in adult healthcare systems. The social worker provides professional services to patients and families, which facilitates the use of individual, public and private resources so that optimal functioning can occur for both the patient and the family. These core services include assessment of psychosocial functioning and capacities, counseling for care planning and decision-making, social work treatment and community resource planning. Social workers collaborate with many disciplines, programs, departments and organizations in the accomplishment of these duties.
