About The Position

CHI St. Alexius Health Williston (Craven Hagan Clinic) in Williston, ND, is seeking a dedicated RN Clinic Navigator to guide patients and their families through their healthcare journey. In this essential role, you will be the cornerstone of comprehensive care coordination, serving as the primary point of contact and ensuring seamless navigation through our health system. You'll work collaboratively with providers and clinical teams to deliver patient-centered solutions.

Requirements

  • A Registered Nurse (RN) with current licensure.
  • Strong collaborative and communication skills to serve as an interdisciplinary team liaison.
  • A compassionate approach to patient care and education.
  • Ability to assess patient needs comprehensively and connect them with vital resources.
  • Associate Of Nursing Associate Degree in Nursing (ADN)
  • 4-6 years Minimum of four (4) years of experience as a RN, upon hire
  • Registered Nurse: ND, upon hire
  • Basic Life Support - CPR, upon hire

Nice To Haves

  • Bachelors Of Science Bachelor’s Degree in Nursing (BSN)
  • 4-6 years Minimum of four (4) years of experience as a RN, upon hire

Responsibilities

  • Facilitate interactions between patients and their clinical care team, ensuring a smooth and coordinated experience throughout the continuum of care.
  • Empower patients, families, and caregivers by educating them about diseases, treatments, side effects, and how to access reliable information.
  • Collaborate with providers to implement care standards, and identify/secure appropriate services. You'll connect patients with crucial resources for psychosocial distress or tangible barriers to care (e.g., finances, housing, transportation), referring to social workers or counselors as needed.
  • Provide resources for specialized treatments or testing not available locally, and promote awareness of appropriate clinical trials.
  • Encourage patients, families, and caregivers to take an active role in managing their care, within the scope of their individual circumstances.
