About The Position

Requirements

• Graduate of an accredited school of nursing.
• RN-Registered Nurse of California.
• BLS-Basic Life Support Healthcare Provider.
• Automobile Insurance.
• Valid Driver's License Class C or B - State Department of Motor Vehicles.
• 5 years recent relevant experience.
• Demonstrated proficient knowledge and competence with regard to nursing theories, concepts and practices; medical terminology; anatomy; and physiology.
• Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.
• Written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
• Proficient knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record (EPIC preferred).
• Ability to prioritize assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.
• Time management and organizational skills, including the ability to prioritize patient care, assignments and work within standardized policies, procedures, and practices to achieve objectives and meet time sensitive activities.
• Ability to work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.
• Ability to identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.
• Ensure the privacy of each patient’s protected health information (PHI).
• Build collaborative relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

Nice To Haves

• EPIC electronic health record experience preferred.

Responsibilities

• Assessment, coordination, delivery, and evaluation of nursing care, including direct patient care and patient/family education.
• Delivery of holistic and individualized care to maximize patient and family comfort and health, consistent with hospice philosophy and Sutter Health policies.
• Development, implementation, management, and coordination of an optimal interdisciplinary plan of care, incorporating psychological, sociocultural, spiritual, economic, and lifestyle factors.
• Fostering and maintaining collaborative relationships with patients, families/support groups, physicians, and other healthcare providers through timely and effective communication.
• Adherence to policies, industry standards, best practices, and applicable laws/regulations/codes to promote a quality, highly reliable patient experience.
• Engagement in continuous growth and development in professional nursing practice.
• Management, storage, preparation, reception, unpacking, transportation, disposal, or administration of hazardous drugs as required by NIOSH and USP 800 guidelines.
