About The Position

Requirements

• Current unrestricted RN licensure in the state of practice.
• At least 1 year of clinical experience.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• For Louisiana: at least 2 years of full-time experience working as an RN. Two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for the required 2 years of RN experience.

Nice To Haves

• Current CPR certification, or ability to complete it within 90 days of hire.

Responsibilities

• Complete an initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify the patient’s and family’s physical, psychosocial, and environmental needs and reassess as needed, at least every 15 days.
• Initiate and coordinate the plan of care while maintaining accurate, relevant clinical notes about the patient’s condition.
• Document problems, appropriate goals, interventions, and the patient’s and family’s response to hospice care.
• Collaborate daily with the patient and family, attending physician, and other members of the interdisciplinary group (IDG) to provide care.
• Evaluate personal and professional performance and make necessary changes to improve productivity and quality of care.
• Adhere to and support hospice policies, procedures, goals, and objectives to assure quality patient care.
