About The Position

Heart of Hospice, part of LHC Group and the Optum family of businesses, provides home health, hospice, and community-based services. The Per Diem Registered Nurse Case Manager assumes full nursing responsibility for delivering the Plan of Care to each hospice patient. The nurse evaluates personal and professional performance and makes changes as needed to improve productivity and care quality, while adhering to and supporting hospice policies, procedures, goals, and objectives.

Requirements

Current unrestricted RN licensure in the state of practice.
At least 1 year of clinical experience.
Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
For Louisiana: at least 2 years of full-time experience working as an RN. Two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for the required 2 years of RN experience.

Nice To Haves

Current CPR certification, or ability to complete it within 90 days of hire.

Responsibilities

Complete an initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
Identify the patient’s and family’s physical, psychosocial, and environmental needs and reassess as needed, at least every 15 days.
Initiate and coordinate the plan of care while maintaining accurate, relevant clinical notes about the patient’s condition.
Document problems, appropriate goals, interventions, and the patient’s and family’s response to hospice care.
Collaborate daily with the patient and family, attending physician, and other members of the interdisciplinary group (IDG) to provide care.
Evaluate personal and professional performance and make necessary changes to improve productivity and quality of care.
Adhere to and support hospice policies, procedures, goals, and objectives to assure quality patient care.
