About The Position

Frontier Hospice is seeking a per diem Hospice Registered Nurse Case Manager to provide end-of-life care throughout the Tulsa community. The role delivers hands-on hospice nursing care, coordinates services, and supports patients and families throughout the hospice journey as part of an interdisciplinary care team. The focus is on compassionate, clinically excellent care that promotes patient comfort, dignity, and quality of life. The position also responds to clinical referrals, helps assess client eligibility and suitability for hospice services in partnership with the Clinical Director, educates patients and families, and encourages their participation in individualized care planning. The RN Case Manager shares call duties with the team.

Requirements

Hold a current, unencumbered Registered Nurse license.
Have at least one year of RN experience in a medical-surgical or acute-care setting.
Be proficient with computers, typing, and various programs, including electronic medical records.
Maintain current CPR certification if required by the state.
Have a current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

Respond to clinical referral information, inquiries, and requests with care and expertise.
Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
Coordinate care and provide direct nursing care to support patient comfort and quality of life.
Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
Educate and advocate for patients and families, encouraging their participation in personalized care plans.
Share responsibility for call duties.
