About The Position
Dierksen Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager to provide end-of-life care throughout the Garland community. The RN Case Manager delivers hands-on nursing care, coordinates services, and guides patients and families through the hospice journey as part of an interdisciplinary care team. The role emphasizes compassionate support, clinical excellence, patient comfort, and quality of life. The nurse responds to clinical referral inquiries, works with the Clinical Director to assess clients’ eligibility and suitability for hospice services, provides direct nursing care, and educates patients and families so they can participate in personalized care planning. The role also shares responsibility for call duties.
Requirements
- Current, unencumbered Registered Nurse license.
- At least one year of RN experience in a medical-surgical or acute-care setting.
- Computer proficiency in typing and various programs, including experience with EMR.
- Current CPR certification if required by the state.
- Current driver’s license and vehicle insurance, with access to dependable transportation.

- Respond to clinical referral information, inquiries, and requests with care and expertise.
