Community Health Coordinator

About The Position

The Community Health Coordinator is responsible for providing health screenings at community events and through our mobile clinic. This role focuses on delivering essential healthcare services to underserved populations in Eastern Kentucky and Southern West Virginia. The successful candidate will play a crucial role in promoting health education, preventing chronic conditions, and improving access to care in rural and remote communities.

Requirements

  • High School diploma or equivalent required.
  • Medical Assistant certification (CMA, RMA, or equivalent) required.
  • CPR/First Aid certification required or must be obtained within 30 days of hire.
  • At least one year of experience in a medical assistant role, preferably in a community health or outreach setting.
  • Experience with health screenings (e.g., blood pressure, glucose, cholesterol)
  • Familiarity with electronic medical record (EMR) systems.
  • Strong interpersonal skills with the ability to effectively communicate with patients from diverse backgrounds.
  • Ability to work independently and as part of a team.
  • Excellent organizational skills, attention to detail, and the ability to multitask.
  • Ability to handle sensitive patient information with confidentiality and professionalism.
  • Willingness to travel to various locations, sometimes in rural or remote areas.

Nice To Haves

  • Experience working with underserved populations or in rural healthcare settings is a plus.
  • Experience in mobile health settings or with mobile clinics is highly advantageous.
  • Knowledge of rural health issues and challenges, particularly in the Appalachian region is preferred.

Responsibilities

  • Assist in performing basic health screenings, such as blood pressure, cholesterol, blood glucose, BMI, and other preventive health checks.
  • Greet patients, explain screening procedures, and ensure comfort and confidentiality.
  • Provide patients with basic health education on results, preventive measures, and healthy lifestyle choices.
  • Accurately record and enter patient information, screening results, and other relevant data into electronic medical records (EMR) systems.
  • Assist with the setup, operation, and breakdown of the mobile clinic van, ensuring equipment and supplies are properly stored and maintained.
  • Work closely with other medical professionals (nurses, physicians, and health educators) to ensure quality care and follow-up for patients.
  • Represent ARH at health fairs, screenings, and other community outreach events, fostering relationships within the local community.
  • Maintain high standards of patient care, safety, and hygiene in all interactions and procedures.
  • Travel to various community locations within the service area, including rural and remote regions, to conduct screenings and health outreach.
  • Other duties as assigned.
