The Assessment Specialist role is part of the Specialized Recovery Support Services program, focusing on assessing individual needs, developing personalized care plans, and connecting them with essential services and supports. Requirements

  • Possesses a Bachelor's Degree in social work, counseling, psychology, or a related field; or Registered Nurse (RN), or Licensed Social Worker (LSW).
  • Possesses a minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years’ post degree experience working with individuals with severe and persistent mental illness.
  • Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.
  • Effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds.
  • Experience working with individuals with chronic conditions or severe and persistent mental illness.
  • Strong knowledge of Medicaid, behavioral health systems, and local community resources.

Responsibilities

  • Maintains and builds knowledge of State and program requirements.
  • Maintains and builds knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
  • Completes in-home visits to assess for eligibility for specified programs and conducts safety assessments, in compliance with CareStar and department guidelines.
  • Documents all applicant and case-related contacts and interventions in the applicant's record per agency and CareStar policy, including standards of content and timeliness.
  • Provides assistance with community resource referrals as appropriate.
  • Adheres to the CareStar Rule in the performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
