About The Position

AdvisaCare is seeking talented Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to join their growing home health team. The role involves helping patients regain strength, improve mobility, reduce pain, prevent falls, and return to activities that matter most to them. AdvisaCare has been helping patients remain safe, strong, and independent in the communities they serve since 1997. This position is ideal for individuals who are skilled in mobility, problem-solving, patient motivation, and making home health visits successful.

Requirements

  • Degree from an accredited PT or PTA program.
  • Current Michigan PT or PTA license in good standing.
  • Strong clinical judgment and problem-solving skills.
  • Excellent communication and patient education skills.
  • Ability to work independently while remaining connected to the care team.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Familiarity with Medicare/CMS documentation preferred.

Nice To Haves

  • Home health experience preferred.

Responsibilities

  • Complete comprehensive evaluations and identify functional needs (PTs).
  • Develop individualized plans of care and treatment goals (PTs).
  • Provide evidence-based interventions and monitor patient progress (PTs).
  • Reassess patients and modify treatment plans as clinically appropriate (PTs).
  • Provide skilled therapy in accordance with physician orders and CMS Conditions of Participation (PTs & PTAs).
  • Deliver therapeutic exercise, gait and balance training, transfer training, strengthening, and functional mobility interventions (PTs & PTAs).
  • Educate patients and caregivers on fall prevention, assistive devices, safety, and Home Exercise Programs (HEPs) (PTs & PTAs).
  • Maintain timely and accurate documentation (PTs & PTAs).
  • Collaborate with physicians, caregivers, and the interdisciplinary team to help ensure successful patient outcomes (PTs & PTAs).
