About The Position

The Advanced Practice Registered Nurse (APRN) – In-Home Clinical Care provides comprehensive clinical assessments, interventions, and care coordination for high-risk health plan members in their homes. The APRN works with Case Management and the interdisciplinary care team to address clinical needs, identify gaps in care, improve treatment and medication adherence, and facilitate access to appropriate healthcare services. The APRN documents visits and care coordination in electronic health records and complies with applicable laws, professional standards, and organizational requirements.
