About The Position

Build your Career. Make a Difference. Presbyterian is hiring a skilled Manager-Disease Prevention and Management. The Manager, Disease Prevention & Management, provides strategic and operational leadership for a multidisciplinary team of health and well-being specialists - including a Program Manager, Health Coaches, Fitness Instructors, and Registered Nurses - focused on advancing disease prevention and management efforts for our clients. This role ensures the delivery of evidence-based programs that improve physical health outcomes for both external and internal employee populations. The Manager oversees program design, implementation, and evaluation to ensure initiatives are data-driven, outcome-focused, and aligned with organizational and client strategic goals.

Requirements

  • Bachelors degree in Public Health, Exercise Physiology, Nutrition, Health Promotion & Health Education, or related field required.
  • 5+ years of experience in health promotion, health and well-being related program management, including staff supervision.
  • Demonstrated success in scheduling, caseload management, and aligning staff skills with program needs.
  • Strong organizational, presentation, and project management skills; proven ability to manage multiple initiatives while meeting deadlines.
  • Excellent interpersonal and communication skills; ability to collaborate effectively at all levels, including senior leadership.
  • Knowledge of health coaching principles, the CDC¿s Diabetes Prevention Program, and other evidence-based well-being practices.
  • Demonstrated ability to mentor, delegate, and lead high-performing teams.
  • Strong understanding of well-being industry trends, program management practices, and evaluation techniques.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint) and well-being-related information systems.

Nice To Haves

  • Masters degree in Public Health, Nutrition, Exercise Physiology, Health Education, or related field.
  • Health Coach Certification and training in Motivational Interviewing strongly preferred.
  • Certified Health Education Specialist (CHES) is preferred.
  • Experience facilitating RFP processes for well-being program service providers a plus.

Responsibilities

  • Direct the development, execution, and continuous improvement of well-being initiatives that are measurable, evidence-based, and outcomes-driven.
  • Ensure programs align with organizational mission, client strategies, and departmental goals; advocate for integrated well-being strategies with clients where none exist.
  • Enhance well-being program engagement on the Personify Health online member platform to strengthen disease prevention/management outcomes and impact.
  • Serve as subject matter expert on disease prevention and management and individual behavior change, providing consultation to internal leaders, clients, and vendor partners.
  • Leverage data and analytics to evaluate program effectiveness, inform decision-making, and support continuous quality improvement.
  • Recommend, implement, and evaluate policies, procedures, and program enhancements that optimize efficiency and sustainability.
  • Participate in client meetings as a consultative partner, contributing to client retention strategies, and the long-term design of workstream offerings.
  • Lead, mentor, and develop well-being specialists (a Program Manager and a PRN pool of Health Coaches, Fitness Instructors, and Registered Nurses) to deliver high-quality, innovative services.
  • Recruit, onboard, and train staff, fostering a culture of collaboration, accountability, and excellence.
  • Manage daily operations, including caseload assignments, workflow management, and event staffing.
  • Oversee documentation of program participation, outcomes, and engagement to ensure compliance and quality standards.
  • Provide coaching, performance evaluations, and professional development opportunities to maintain a high-caliber team.
  • Support staff in administrative processes, including scheduling, expense management, and goal setting.
  • Oversee disease prevention and management programs such as biometric screenings, etc.
  • Interpret and communicate biometric screening results, ensuring participants understand their health data and next steps.
  • Coordinate and execute online well-being campaigns across nutrition, health coaching, and exercise science activities.
  • Utilize the Program Manager(s) and Well-being Coordinators to refine program objectives and improve participant engagement.
  • Evaluate and enhance organizational well-being-related policies for clients (e.g., tobacco use, nutrition standards, vending and cafeteria policies).
  • Perform other duties as assigned.
