About The Position

Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated LPN Coordinator/Dispatcher to work in their West Palm Beach location. This role involves providing coordination and dispatch of staff visits on a Monday - Friday 8:00 am – 4:30 pm schedule. Trustbridge Hospice has been serving families across Palm Beach and Broward counties with compassionate, community-based care since 1978, continuing the legacy of Hospice by the Sea and Hospice of Palm Beach County to provide comfort, dignity, and peace to those facing advanced illness.

Requirements

  • Current license to practice as a Licensed Practical Nurse in the State of Florida.
  • Current Basic Life Support Certification.
  • Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
  • Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.
  • Basic computer skills and EMR software experience a plus.

Responsibilities

  • Coordinating with Managers and team members in patient/family direct and indirect care needs.
  • Providing quality professional intervention based on protocols and policy regarding the physical, emotional and psychological needs.
  • Interacting with management, physicians and field staff utilizing a teamwork approach.
  • Serving as a professional leader in assessing, planning, guiding and evaluating patient care activity.
  • Pledging to make the comfort and care of patients and families the highest priority.
  • Consistently providing a high level of personalized, sincere and engaging service to patients and families at all times.
  • Ensuring the complete satisfaction of the patient and family.
