About The Position

Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated LPN Coordinator/Dispatcher to work in their West Palm Beach location. This role involves providing coordination and dispatch of staff visits on a Monday - Friday 8:00 am – 4:30 pm schedule. Trustbridge Hospice has been serving families across Palm Beach and Broward counties with compassionate, community-based care since 1978, continuing the legacy of Hospice by the Sea and Hospice of Palm Beach County to provide comfort, dignity, and peace to those facing advanced illness.

Requirements

Current license to practice as a Licensed Practical Nurse in the State of Florida.
Current Basic Life Support Certification.
Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing.
Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs.
Basic computer skills and EMR software experience a plus.

Responsibilities

Coordinating with Managers and team members in patient/family direct and indirect care needs.
Providing quality professional intervention based on protocols and policy regarding the physical, emotional and psychological needs.
Interacting with management, physicians and field staff utilizing a teamwork approach.
Serving as a professional leader in assessing, planning, guiding and evaluating patient care activity.
Pledging to make the comfort and care of patients and families the highest priority.
Consistently providing a high level of personalized, sincere and engaging service to patients and families at all times.
Ensuring the complete satisfaction of the patient and family.
