Requirements

  • Bachelor’s degree in social services or related field with at least two years' experience in casework, clinical or direct services with children; demonstrated ability for Social Services and Casework management; and qualify for Mental Health Professional under Illinois Department of Mental Health Rule 132.
  • Trauma-informed: Must display a willingness to adopt trauma informed interventions, principles, and practices as well as commitment to ongoing development of their capacity for application of trauma informed care. Realizes the prevalence of trauma among persons served by Caritas and the general population. Recognizes how trauma affects everyone involved-including clients, their families, staff, and community; and impacts behaviors. Responds to trauma by actively implementing the guiding principles of trauma informed care into their daily practice. Resists re-traumatization of persons served and staff providing services, by creating safe, welcoming, and supportive environments.

Responsibilities

  • Serves as the primary point of contact for all communication for assigned youth.
  • Coordinates, schedules, and leads staffing’s (pre-admission, admission, monthly, discharge, and aftercare) for all assigned youth. Completes requires CSR documentation within 24 hours in the state RedCap system.
  • Coordinates all school related activities for assigned youth, to include enrollment, attendance to IEP meetings, and all necessary communications with schools.
  • Advocates for the educational, medical, social, emotional, and physical needs of the assigned youth to the various members of youth’s teams.
  • Coordinates and schedules all visits/passes for assigned youth.
  • Documents all client related communications within the ACE database.
  • Completes all required 906 documentation for assigned youth within the required timelines.
  • Attends phone conferences and training to keep current on State requirements and best practice in the field.
  • Completes documentation related to Medicaid service provision in a manner that is compliant with Medicaid billing standards.
  • Provides direct services and crisis intervention work to residents in daily activities as needed.
  • Attends weekly supervision with Residential Service Coordination Supervisor.
