About The Position

The Youth Outreach Case Manager provides holistic case management and outreach services to disconnected youth ages 18–24 experiencing homelessness. The role identifies and conducts outreach to homeless youth in the Eastern Panhandle and coordinates appropriate community services to support participant success. The position involves interaction with participants, community resources, staff members, volunteers, and other agency representatives.

Requirements

Associate degree in Human Services and 1–2 years of experience working with youth or young adults; additional experience may substitute for the education requirement.
Valid driver's license and continual access to a vehicle.
Experience providing holistic case management services to youth or young adults.
Experience interacting with volunteers.
Knowledge of Eastern Panhandle social services networks.

Responsibilities

Conduct screenings and intakes.
Engage in regular outreach to homeless youth.
Analyze youth needs and available resources.
Complete needs assessments for all eligible youth.
Assist young adults in developing goals and objectives and formulating action plans.
Provide case management services, including advocacy and referrals.
Locate and refer clients to safe and affordable housing.
Maintain accurate and timely case records, reports, and statistical data.
Interact with low-income populations.
Maintain confidentiality within and outside the agency.
Work independently, using initiative, creativity, and flexibility to meet participant needs while preserving their dignity.
Perform other duties as assigned.
