The Healthy Relationship Program provides flexible, community-based services tailored to families’ needs, including intensive family engagement, evidence-based education, and mentoring. Youth Coaches work directly with youth to reduce the risk of out-of-home placements. They provide one-on-one mentorship that builds on young people’s strengths, fosters connections with school, community, and family, and supports the development of achievable goals. Coaches help youth build independent life skills, access services and opportunities, and establish supportive networks. They draw on familiarity with the communities they serve and an understanding of how Social Determinants of Health affect quality of life and risk to provide relevant support.
