The Wraparound Facilitator provides individualized, coordinated, family-driven care for youth and families with complex needs. The role facilitates a high-fidelity wraparound process for youth involved with multiple child- and family-serving systems, at risk of out-of-home placement, and experiencing behavioral or mental health difficulties. The facilitator brings the youth, family, providers, and members of the family's social support network together to develop a plan of care responsive to the youth's needs. The position works with an Independent Evaluator (Licensed Mental Health Professional) to ensure a comprehensive assessment is completed and incorporated into the Child and Family Team process. In partnership with the family, the facilitator develops an individualized Plan of Care consistent with National Wraparound Initiative standards and applicable treatment-planning requirements. The role also helps youth and families access strength-based mental health, social, educational, and other services and supports. Eckerd Connects' Wraparound Agencies participate in Louisiana's Coordinated System of Care (CSoC), a behavioral health managed care program serving people with behavioral health, mental health, and addictive disorders. The program coordinates flexible services and supports across systems, builds meaningful partnerships with families and youth, and provides family-driven, youth-guided services in homes and community settings. Plans are individualized, strength-based, culturally and linguistically competent, and integrated across child-serving systems.
