About The Position

The Wraparound Facilitator provides individualized, coordinated, family-driven care for youth and families with complex needs who are involved with multiple child- and family-serving systems, at risk of out-of-home placement, or experiencing behavioral and mental health difficulties. The facilitator coordinates youth, families, providers, and members of the family’s social support network to develop a plan of care responsive to the youth’s needs. In collaboration with an Independent Evaluator who is a Licensed Mental Health Professional, the facilitator helps ensure a comprehensive assessment is completed and incorporated into the Child and Family Team process. In partnership with the family, the facilitator develops an individualized Plan of Care consistent with National Wraparound Initiative standards and applicable treatment-planning requirements. The facilitator also helps children, youth, and families access strength-based mental health, social, educational, and other services and supports to help them achieve their vision. Eckerd Connects’ Wraparound Agencies participate in Louisiana’s Coordinated System of Care, a behavioral health managed-care program for people with behavioral health, mental health, and addictive disorders. The program uses a family-driven, youth-guided model to coordinate individualized, strength-based, culturally and linguistically competent services across systems. Services are provided in homes and community settings and are data-driven and outcome-oriented.
