About The Position

The Wraparound Facilitator position is full-time and hybrid, combining remote work from a home office with community-based meetings several times a week. The role serves Miami County, Indiana, and applicants must reside within the county and be able to travel throughout it as needed. Travel may take up to an hour and a half for meetings.

The facilitator works with emotionally and/or behaviorally challenged children and their families to build a support team, develop a plan based on the family’s strengths and needs, connect families with services and supports, and facilitate regular team meetings to review the plan of care.

Requirements

• Bachelor of Arts or Bachelor of Science degree.
• At least 24 months of experience in one or more of the following: clinical work, case management, skills building, child welfare, juvenile justice, or education in a K-12 setting.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Residence within Miami County, Indiana, and ability to travel throughout the county for required in-person meetings.
• Designated home office setting with a computer, internet service, and smartphone.

Responsibilities

• Help families develop a wraparound team of caring professionals, friends, and family.
• Create a wraparound plan based on the strengths and needs of the family.
• Help families access services and supports needed to meet their goals.
• Facilitate regular community-based wraparound team meetings to review the plan of care.
