About The Position

The Wraparound Facilitator serves Cass County, Indiana, and surrounding counties, working with emotionally and/or behaviorally challenged children and their families. This full-time, hybrid position combines remote work from a home office with travel of up to an hour and a half for community-based meetings several times a week. Applicants must reside within Cass County, Indiana, and be able to travel throughout the county for required in-person meetings. The facilitator helps families build a supportive team, create a plan based on family strengths and needs, connect with services and supports, and review the plan of care in regular team meetings.

Requirements

• A BA/BS degree.
• At least 24 months of experience in one or more of the following: clinical work, case management, skills building, child welfare, juvenile justice, or education in a K-12 setting.
• Must reside within Cass County, Indiana.
• Ability to travel throughout the county for community-based meetings several times a week, with travel of up to an hour and a half.
• Valid driver's license, car insurance, and reliable transportation.
• Ability to pass thorough background screens.
• Designated home office setting with a computer, internet service, and smartphone.

Responsibilities

• Help families develop a wraparound team consisting of caring professionals, friends, and family.
• Create a wraparound plan based on the strengths and needs of the family.
• Help families access services and supports needed to meet their goals.
• Facilitate regular community-based wraparound team meetings to review the plan of care.
