The Workforce Programs Case Coordinator is a full-time member of The New Jewish Home’s SkillSpring Department, working with participants in young adult and high school programs to help them prepare for and secure employment. The role uses a care management process: assessing participants’ functional, physical, cognitive, and social-emotional status and support systems; developing care plans; arranging coordinated services; monitoring changes and responses to interventions; and reassessing needs regularly. The coordinator also serves as a coach and advocate, helping participants address their needs with compassion and grace. The role requires a strong work ethic, a passion for working with underserved young adults, and the ability to build and manage relationships with internal and external partners. In collaboration with the Workforce team, the coordinator facilitates young adult programming, including professional development, clinical skills training, and health career exploration; monitors participation and program data; and supports other activities as needed. The coordinator helps create a diverse, culturally sensitive environment relevant to young adults. The schedule may include some mornings, evenings, and Saturdays at multiple sites; hours, days, and locations fluctuate with seasonal changes.
