About The Position

As a Wellness Nurse LPN at Sunrise Senior Living, you monitor resident health, support medication oversight, and maintain clinical quality across the community. You help prevent decline through proactive assessment, early intervention, and strong clinical oversight. The role partners with families, physicians, and the Resident Care Director and supports care staff with documentation and medication practices.

Requirements

• Active LPN license in good standing.
• Graduate of an approved nursing program.
• At least 1 year of experience in a senior care or clinical setting.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Conduct move-in, 3-day, semi-annual, and change-of-condition assessments.
• Perform monthly wellness visits and monitor health trends.
• Follow up on falls, incidents, and hospital returns.
• Transcribe physician orders and ensure accurate MAR/TAR documentation.
• Complete medication audits and oversee medication compliance.
• Partner with families, physicians, and the Resident Care Director.
• Support infection control and regulatory standards.
• Educate and guide care staff on documentation and medication practices.
