About The Position

BJC Home Care is a regional home care network serving patients in Missouri and Illinois. It offers skilled nursing, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment, high-tech respiratory care, and specialty home care programs.

The Triage Nurse takes after-hours calls from clients and evaluates, counsels, and furnishes hospice, supportive care, or other home health services. This is a PRN, as-needed role with flexibility for day and night shifts and a need for weekend coverage. The role is not benefit eligible.

Requirements

• Nursing diploma or associate's degree.
• 2–5 years of experience.
• Registered Nurse (RN) license.
• RN license in Missouri and Illinois.

Nice To Haves

• Bachelor's degree in Nursing.
• 5–10 years of experience.
• Cardiopulmonary Resuscitation certification.
• Hospice and home health experience.

Responsibilities

• Provides patient care counseling under the direction of the physician.
• Receives after-hours calls directly from hospice and supportive care patients and assesses patient information to determine whether a visit is needed.
• Documents care given and time spent on the phone for hospice and supportive care triage calls.
• Tracks patient problems and trends information for staff to identify areas for education.
