About The Position

Passavant Memorial Homes is seeking a full-time Triage Nurse to perform selected nursing acts for individuals with intellectual disabilities, autism, and behavioral health needs. The role is supervised directly by the Program Operations Director for the Rochester Region and is based in-office at the Rochester location. The Triage Nurse provides care in a safe, competent, and humane manner and supports individuals’ health needs in community residential settings.

Requirements

Graduate of an accredited school of professional and/or practical nursing.
Current nursing license by examination from the State of Pennsylvania.
Valid Pennsylvania driver’s license.

Nice To Haves

Understanding of the intellectual disabilities field.
One to two years of experience in medical/surgical or geriatric nursing.

Responsibilities

Perform clinical nursing skills using knowledge and understanding of nursing fundamentals.
Monitor community residential settings on-site as required.
Monitor staff completion of new medication orders and documentation.
Provide ongoing assessment and planning to ensure individuals’ health needs are met.
Administer injections as needed, including Depo-Provera shots, Hepatitis B vaccines, and TDap boosters.
Complete dietary in-service training for specialized diets and send the materials to community homes.
File laboratory results, physicians’ orders, consults, and other paperwork related to individuals served.
Communicate daily with hospital staff and provide updates to the Clinical Services Director and Regional Program Directors.
Complete other duties as assigned by the Clinical Services Director.
