About The Position

Good Shepherd Community Care, a community-based, mission-driven hospice and palliative care organization named one of the Top Places to Work in Massachusetts for the last four years, seeks a Part-time Weekend Triage Nurse to join its growing weekend team. This is a 20-hour position that requires visit availability. The Weekend Triage Nurse will be responsible for ensuring care for new and existing hospice patients on Saturdays and Sundays.

Requirements

• Licensed Registered Nurse in the State of Massachusetts
• Good communication skills
• Demonstrated ability to work with a team
• Strong critical thinking skills
• Triage experience
• Experience working with EMRs
• Excellent organizational and time management skills

Nice To Haves

• 2 years or more of hospice experience is preferred.
• Experience with Google Suite preferred.

Responsibilities

• Preparing for the weekend, including reviewing clinicals for admissions and preparing the weekend schedule.
• Triage patient calls, including providing clinical recommendations, distributing visits to nurses and providing support to patients and caregivers.
• Support clinical team in the field, providing information and coordinating resources.
• On occasion, help with clinical visits as needed.
• Coordinate patient care and handoff from the weekend to the weekday staff.
