About The Position

Requirements

Active Registered Nurse license
Ability to work in a skilled nursing facility setting
Availability for approximately 11 weeks with guaranteed 40 hours per week
Strong communication and collaboration skills; proficient documentation in electronic records

Experience in utilization management or care coordination is preferred

Responsibilities

Review patient records to determine appropriate level of care and utilization in line with guidelines and payer requirements
Collaborate with physicians, nursing, and care coordination teams to develop and adjust care plans
Participate in discharge planning and transition activities to support appropriate post-acute care
Document utilization decisions in the electronic health record and relay changes to the care team
Monitor patient progress, identify potential utilization issues, and escalate as needed
Contribute to quality improvement initiatives and ensure compliance with regulatory and facility policies
