About The Position

This is a travel assignment for a Registered Nurse in Case Management in Saint Helena, CA, starting on 09/14/2026 for a duration of 13 weeks. The role involves supporting patient care in patients' homes within a home health and hospice setting. The nurse will manage a caseload, perform assessments, provide direct and indirect nursing care, educate patients and families, and coordinate care with other disciplines. The position requires a Registered Nurse license in California, CPR/BLS certification, at least two years of recent experience, and EPIC experience. A Bachelor's Degree in Nursing (BSN) is preferred. The role also requires a valid Driver's License, being at least 21 years old, current automobile insurance, and a reliable automobile.

Requirements

  • Registered Nurse (RN) license in the state of practice (California) required

Responsibilities

  • Plan, organize and direct home care services to meet patients’ needs in their homes; manage a patient caseload effectively.
  • Perform initial and ongoing nursing assessments; document patient status, response to treatment, and plan of care.
  • Provide direct and/or indirect skilled nursing care within the plan of care; counsel and educate patient and family on nursing and related needs.
  • Update nursing plans, medication profiles, and documentation promptly; ensure timely submission of all records per policy.
  • Coordinate patient care with other disciplines; incorporate input from the care team and adjust the plan of care as goals are clarified.
  • Communicate changes to goals or status with the patient and family, and serve as a resource to patients, families, and other care team members.
