About The Position
This is a 13-week travel assignment for a Registered Nurse Case Manager in Chillicothe, Ohio. The role is hospital-based, focusing on acute medical and surgical patients. The assignment offers a dayshift schedule with guaranteed hours and no on-call, holiday, or weekend work. The weekly pay range is estimated between $2,376–$2,506.
Requirements
- 3+ years Acute Care Case Management experience
- Must reside outside a 60-mile radius of the assignment location
- Ability to work in a hospital-based setting
- Strong communication and collaboration skills
- Experience with discharge planning and addressing social determinants of health
- Proficiency in clinical documentation and care coordination
Responsibilities
- Assess discharge needs, social determinants of health, and discharge planning/referrals for Acute Medical and Surgical patients.
- Develop and implement safe, timely discharge plans and coordinate referrals to post-acute services.
- Collaborate with an interdisciplinary care team to support smooth patient transitions.
- Document care coordination activities in the patient record with accuracy and compliance.
- Manage a typical caseload of around 20 patients per day, ensuring timely discharge planning.
- Communicate with patients, families, and care partners; provide education and resource linkage to support ongoing care.
