About The Position

This is an 8-week travel temporary assignment for a Registered Nurse, Case Management in Grand Junction, Colorado. The role requires 36 guaranteed hours per week and involves weekend coverage. The position operates in an inpatient care setting within an interdisciplinary care team. The primary focus is on developing, implementing, and evaluating the Case Management Plan to meet patients’ health and human service needs. The RN Case Manager will provide advocacy, coordination, and education while ensuring care aligns with regulatory and payer requirements. This is a full-time position with a quick start urgency.

Requirements

• Current, unrestricted Registered Nurse license in Colorado
• Bachelor’s Degree in Nursing
• At least three (3) years of clinical experience in a hospital setting
• At least five (5) years of case management experience in a hospital setting
• Ability to document and communicate clearly
• Strong collaboration with physicians, patients, families, and staff
• Willingness to fulfill weekend coverage and participate in the described weekend rotation

Nice To Haves

• Master’s degree preferred
• Case management certification preferred or willingness to obtain within two (2) years of employment

Responsibilities

• Develop, implement, and evaluate case management plans for patient populations, coordinating care across disciplines.
• Advocate for patients and families; communicate and collaborate with physicians, nurses, social workers, and payers to optimize care.
• Document Case Management plans clearly, including all patient contacts and care decisions.
• Facilitate appropriate utilization and discharge planning; participate in interdepartmental meetings related to Case Management/Social Services.
• Collaborate with payers and review organizations to ensure proper resource use and adherence to regulatory requirements.
• Maintain current knowledge of discharge planning, utilization processes, and case management best practices through ongoing education.
